Own or pooled prevention service: you must attach the articles of incorporation.

Main reasons for invalidation:

 

The document does not show the date until which you are paid up.

The document does not define the specialties contracted: these must be hygiene, safety, ergonomics, psycho-sociology and health surveillance.

The Tax ID does not match that given when registering on the platform.
